Double Mastectomy — Top Surgery
If you are considering Top Surgery, a Double Incision Mastectomy may be the best option for those with medium to large chest sizes. Your surgeon will make two flat incisions on your chest, one above and another under your pectoral muscle. The actual muscle will be left alone, but Mr Vahidi will pull back the skin to remove your breast tissue.
He may use liposuction aswell to create a more masculine contour and to prevent dog ears. This can be done as part of the surgery or as a separate male chest contouring procedure.
Peri-Areolar Top Surgery
Peri-Areolar Top Surgery is a suitable option for individuals with a small chest up to a B cup size who desire a more masculine appearance. During the procedure, two incisions are made around the perimeter of the areola; the first one is a smaller circle, while the second incision is a larger concentric circle.
The excess skin between the two incisions is removed, as well as the breast tissue. Liposuction may also be used to enhance the chest contour. This surgical technique can help achieve a flatter, more masculine-looking chest.
Thanks to the peri-areolar technique, the nipple stalk is usually preserved, and in some cases, resized. This approach also helps maintain sensation as the nipple stalk and nerve are not severed.

Lipomodelling
Lipomodelling, also known as lipofilling or fat grafting, is a cosmetic procedure that involves the removal of fat (liposuction) from one part of the body and transferring it to another. This procedure is commonly used to enhance the appearance and texture of breasts, either after reconstruction or as a standalone cosmetic procedure. With lipomodelling, patients can achieve natural-looking results as the procedure uses the patient’s own fat cells.
The process of transferring fat to breasts involves two stages.
Firstly, fat is extracted from areas such as your thighs, buttocks, stomach, and perhaps your flanks (hips) through liposuction.
Secondly, the extracted fat is purified by separating it from blood and other fluids before being re-injected into your breasts. This procedure helps to enhance the shape and volume of the breasts.
Liposuction and re-injection using fine needles leave minimal marks and no visible scarring. This procedure contours the breast with natural-looking results, which are subtler than those achieved through silicone implants.
Additionally, the removal of fat from certain areas of the body creates a slimmer finish. Often if you’re looking for an increase in cup size, then this is a recommended treatment as it doens’t involve surgery. But, In terms of transgender surgery, this procedure is generally used after you’ve had a mastectomy to improve the contours of your chest so the shape doesn’t have any irregularities or dimples.

Top Surgery Post-Operative Care
After surgery, both top surgery, liposuction/ body contouring or implants, patients are typically fitted with two small tubes called drains that run along the incision site and come out through a small incision under your arm. These drains connect to a small collection tank to gather any excess blood or fluid. They usually leave the drains in place for five days, so you’ll need to empty them several times a day.
After undergoing top surgery, you’ll need to wear a compression binder for up to six weeks. This binder prevents fluid buildup, promote skin adherence to the chest wall, and reduce scarring. It is an essential tool for post-surgery recovery and helps ensure the best possible outcome for you as our patient.

The healing process after treatment can differ depending on the patient, but typically, individuals can resume light work activities within two weeks. However, it is important to avoid any strenuous activity such as heavy lifting, lifting your arms above your head, or weight training for a period of 6-8 weeks to ensure proper healing and avoid any complications. We’ll advise you in conjunction with your post surgery appointments at the clinic on the best course of action for your particular circumstances.
What's the procedure for booking in with Essential Feeling?
As a Transgender Clinic patient you don’t need to do anything. When you book your surgery with Transgender Clinic they will automatically book us so that you have an MLD massage once you are discharged, and then at your five day and two week check up appointments. If you want anymore than this, then we do advise this. So, if you are in the London or Essex area then we’d love to see you privately as well. But if you’re not, once you have had sessions with us, you know what to ask for when booking appointments with therapists who are more local to where you live.
